Testosterone replacement therapy (TRT) is a medically supervised treatment that restores testosterone levels in men diagnosed with low testosterone (hypogonadism). It's administered via injections, topical creams, or pellets depending on the patient's needs. What are the symptoms of low testosterone in men?
Common symptoms of low testosterone include: persistent fatigue and low energy even with adequate sleep, decreased libido and erectile dysfunction, difficulty building or maintaining muscle, increased body fat (especially around the midsection), brain fog, poor concentration, and irritability, depressed mood or loss of motivation, and disrupted sleep patterns. What causes low testosterone?
Low testosterone can result from primary hypogonadism (a problem with the testes — from injury, infection, or genetic conditions like Klinefelter syndrome), secondary hypogonadism (a problem with the pituitary or hypothalamus signaling), or mixed/functional causes which are the most common. Functional causes include aging (testosterone declines roughly 1–2% per year after 30), obesity, chronic stress, poor sleep, certain medications (like opioids or corticosteroids), and metabolic conditions like type 2 diabetes. During your consultation, we review your labs and history to identify the root cause — not just treat the number.
How do I know if I have low testosterone? What labs are needed?
The only way to confirm low testosterone is through bloodwork. We order a comprehensive hormone panel that includes total testosterone, free testosterone, SHBG, estradiol, LH, FSH, CBC (including hematocrit), metabolic panel, lipids, PSA, and thyroid function. Labs should be drawn fasting, in the morning when testosterone peaks. If you've had labs done in the past 3 months, bring them — we'll review what you already have and only order what's missing. We coordinate with Quest Diagnostics, LabCorp, your local hospital lab, or can ship an at-home collection kit directly to you.
At what testosterone level is TRT recommended?
There's no single cutoff that determines treatment. Most guidelines consider total testosterone below 300 ng/dL as clinically low, but symptoms matter just as much as numbers. Some men feel terrible at 350 while others function fine at 280. At ZYP, we look at the whole picture — total testosterone, free testosterone, SHBG, symptoms, age, and health history. The goal isn't to chase a number on paper; it's to find the level where you feel and function your best while staying within a safe, physiologic range.
What is the goal of TRT? What testosterone level should I target?
The goal is to restore testosterone to a level where your symptoms resolve and you feel like yourself again — typically somewhere in the 600–900 ng/dL range, though this varies by individual. We also monitor free testosterone, estradiol, and hematocrit to make sure everything stays balanced. We start conservative, check labs at regular intervals, and adjust dosing based on how you feel and what your bloodwork shows. This is a partnership — you know your body, we know the medicine.
How is testosterone administered? Do I have to inject myself?
We offer multiple options: Testosterone cypionate injections are the most common — self-administered at home using a small subcutaneous or intramuscular needle, typically once or twice per week. Most guys are nervous at first but it becomes routine within a week. We walk you through the process during your consultation. If needles aren't your thing, we also offer testosterone cream applied daily. We also have natural approaches like enclomiphene and our Elevate-T formula (DHEA + enclomiphene + pregnenolone) that stimulate your body's own production without exogenous testosterone.
How long does it take to feel the effects of TRT?
Most men notice improved energy and mood within 2–3 weeks. Libido improvements often follow by weeks 3–6. Changes in body composition (less fat, more muscle) take longer — typically 8–12 weeks with consistent training and nutrition. Full stabilization usually occurs around 6–8 weeks, which is when we recheck labs and make dosing adjustments. Everyone responds differently, so patience and consistent dosing are key in the early weeks.
Can I stay fertile while on TRT?
This is critical and something many clinics overlook. Exogenous testosterone suppresses sperm production by shutting down LH and FSH signaling. If you're planning to have children, we add clomiphene or enclomiphene to your protocol to maintain fertility while on TRT — included in our TRT + Fertility Protection plan ($179/mo). Alternatively, enclomiphene alone can raise testosterone while preserving and often increasing sperm counts. We discuss fertility at every initial consultation.
What happens if I stop TRT?
Your testosterone will gradually return toward pre-treatment levels over several weeks. During this period, you may experience a temporary dip below baseline as natural production restarts — fatigue, low mood, and reduced libido can return. We don't recommend stopping abruptly. If you decide TRT isn't for you, we'll taper your dose and may use clomiphene to restart natural production smoothly. This is another reason we take an education-first approach before starting — we want you to understand the commitment.

What are the common side effects of TRT?
Most side effects are mild and manageable: acne or oily skin (especially early on), elevated hematocrit (we monitor closely via CBC), elevated estradiol causing water retention or nipple sensitivity, testicular atrophy if fertility protection isn't used, and mild injection site reactions. Serious complications are rare with proper monitoring — which is exactly why we check labs regularly and adjust dosing based on your response, not a one-size-fits-all protocol.
What if my hematocrit gets above 54% on TRT?
Elevated hematocrit increases blood viscosity and clotting risk — it's one of the most important markers we monitor. If yours starts trending up, we have several approaches: adjusting dose or frequency, switching to cream (less hematocrit elevation), therapeutic phlebotomy (donating blood), increasing hydration, or adding naringin. We monitor CBC at every lab draw and address trends early — not after they've become a problem.
What are symptoms of elevated estradiol? How is it treated?
Testosterone converts to estradiol via aromatase — levels can climb, especially with higher body fat. Symptoms include: water retention and bloating, mood swings or irritability, nipple sensitivity or gynecomastia, paradoxically decreased libido, and difficulty losing weight. Treatment options include adjusting dose or injection frequency, anastrozole (used judiciously), and lifestyle modifications like reducing body fat and limiting alcohol. We monitor estradiol at every lab check.
Is TRT safe long-term? Does it cause heart problems or prostate cancer?
Current evidence suggests TRT at physiologic doses, when properly monitored, is not associated with increased cardiovascular risk — and may improve cardiovascular markers in men with low testosterone. The TRAVERSE trial (2023), the largest TRT safety study to date, found no increased risk of major cardiac events. Regarding prostate cancer, TRT does not cause prostate cancer based on current evidence, though it's contraindicated in men with active prostate cancer. We monitor PSA at baseline and regularly on treatment.
